Please note: Some insurance companies require a referral to be approved by the insurance company before you can see a specialist. Please make sure you know the requirements of your insurance prior to your appointment to prevent rescheduling. If you are seeing one of our providers for an ear- related issue, you will need to call the clinic to schedule an appointment. For all ear appointments you will also be scheduled to see an audiologist and we need to make sure we have that availability. Thank you.

If you need to see the doctor regarding your ears, please call the office directly as you will also need to be scheduled with an audiologist for a hearing test.

If you are unable to find a desired time, please call us directly and the receptionist will be happy to accommodate you.In order to expedite your visit, please bring the following items:
• A list of current medications • Insurance card(s) and a Photo ID
Please be prepared to pay your co-pay and $25.00 co-insurance at the time of your visit if you have a co-pay and coinsurance.
